Instructions for filling out the PDF application for charter.

The application is separated into four sections.   Each PDF must be downloaded and completed.  Note that the files can not be saved and can only be printed once filled in.   The PDF for each section can be downloaded at the bottom of the page.

• Open with Adode Acrobat reader and Enter The Chapter name where indicated and print.

 Part I 

• Open with Adobe Acrobat reader and print. (This page must be printed and signed by each chapter member)

Part II

• Open with Adobe Acrobat Reader.
• Fill in each section completely.
• Click print button on form to print out completed form 
• Suggest you print several copies, one to submit, and one for your records as the data will not be saved.

Part III           

• Open With Adobe Acrobat Reader.
• Each of the questions in Part III require an extended amont of info
• Suggest you print the form out prior to answering any questions.
• Create a word doucment with your responses. (This will allow these to be answered over a period of time, as the PDF file can not be saved.
• Copy and paste your word document answers into the spaces provided on the adobe form.

Print the completed Document.
